- California Probate Code Section 13101 Declaration for PERSONAL Property of less than $150,000;
- California Advance Health Care Directive (commonly referred to as the “extraordinary means” decision form) and appointment of someone you trust to tell your wishes to your treating physicians;
- California Power of Attorney (which gives someone you trust the power to administer your affairs while you are alive — you chose whether it takes effect immediately or after you are mentally and/or physically unable to take care of your daily affairs);
- California Statutory Will;
- Do Not Resuscitate (must be on pink paper).
- HIPAA Privacy Authorization Form
- Advanced Health Care Directive (It’s a walk yourself through a directive to help people talk to their loved ones about what decisions they would make if they were able before they are not able and the agent doesn’t know what to do).
